About the exam
Step 1 is the first of the three United States Medical Licensing Examination steps, taken by medical students and graduates — international as well as US — who are pursuing US residency training. It assesses how well you apply basic-science concepts to clinical situations: mechanism-of-disease reasoning, pharmacology, pathology, and behavioral science, presented almost entirely as patient-based multiple-choice vignettes.

Is USMLE Step 1 pass/fail?
Yes. Step 1 transitioned to a pass/fail score reporting system, which shifted strategic weight toward Step 2 CK for residency applications. Passing still requires serious preparation — the exam remains a comprehensive gatekeeper of basic-science competence.
How many questions are on Step 1?
The exam is delivered as a multi-section day of roughly 280 multiple-choice items divided into seven blocks of up to forty questions each, with limited break time. Training with full-length timed blocks is essential conditioning.
When should I take Step 1?
Most students sit the exam after completing preclinical coursework, commonly following or overlapping their second year of medical school. Coordinate timing with your school’s requirements and leave room in your schedule for a dedicated preparation block.
